Pulmonary rehabilitation is among the most effective non-drug treatments for chronic lung disease. But coverage stops after a short course of sessions, and when it does, patients lose the structure, the schedule and the accountability that were keeping them well.
Participation falls off at exactly the point where sustaining the gains matters most, and the clinic loses visibility at the same moment it loses contact.

Safety checks built in
Set two alert levels for vitals or RPE. Patients get an immediate warning, or are told to stop exercising, when a reading crosses your limits, and your team is alerted.

Our mission
Make long-term care the default for chronic disease.
Exercise is the single most important factor in life expectancy that doesn’t come in a pill, and structured rehab is one of the most effective ways to deliver it. Yet for most patients, that support lasts only weeks.
We’re building the infrastructure that lets clinics keep providing it for as long as patients need it, and get reimbursed for doing so. We’re starting with pulmonary rehabilitation. The same model can serve every chronic condition that depends on continuous care.
